I found, that as a doctor, the main character should have been able to at least try not to self medicate as she was well aware of the potential side effects and knew she needed a clearer head. As with Girl on the train I find it hard to relate to someone who is such a heavy drinker although I realize this is a way to show the unreliability of the narrator and the seriousness of their current plight and emotional state.
I also found that it was easy to figure out who was responsible even if you weren't entirely aware of all the details and reasons behind their actions. As a child psychologist shouldn't Anna have been able to see past Ethan's facade? She was supposedly very good at her job.
As a side, if you are going to use a pet cat as a way to know who did it then please give an update on said cat in the epilogue.
