Rarely have I seen a movie that stayed so faithful to the book. Of the three notable differences I discerned, two were significant. In the book, it is Mary Crane who steals the money. In the movie, her name is Marion. So insignificant, I can't even understand why they changed it. A significant and shocking difference is in the shower scene. In the book, Mary is not only stabbed, but also beheaded.
I guess that was a little too gory for Alfred Hitchcock. The other significant change is in the Norman Bates character. I think most people are familiar with Anthony Perkins, the perfectly creepy Norman Bates. Tall, thin, neat, nervously fidgety. However, in the book, Norman is depicted as being fat, bespectacled, disheveled bordering on slovenly, and a heavy drinker. I, for one, will always think of Tony Perkins as the real Norman Bates.

This was written in 1959, but it holds up well to the test of time.
