challenging dark sad tense

3.5 stars? I've gone back and forth with myself on this book, this series. Do I like it? Do I hate it? I am honestly not sure (not unlike the characters in the books). The love/hate is strong in this one to the point of pain, but I was gripped and couldn't look away and I woke up at 2:30 this morning because I needed to dive into book two so take what you will from that. This is much darker than my usual read. Normally in the "dark" books I read, there are dark moments but the characters love each other in the end. I am not sure that will ever happen. The feelings are powerful, but they swing so wildly between hate, lust, rage, and sorrow the reader can scarcely keep up. All with the undertow of the setting and the potential unreliability of the narrators minds in play as well.  
So many CW for this one.
Sexual assault on page, gRape on page, violent assault on page, mistreatment of patients, abuse of power, drug use, very borderline noncon/dubcon scenes between MMCs (I found all the smut scenes between the MMCs challenging to read), breath play, blood play, self harm on page, mutual self-harm, suicidal ideation and attempts on page.
This is still not an exhaustive list, make sure you read the author's note before you proceed if you have triggers.
