Without getting spoilery, I have a hard time with the thought that a person with Asperger's would find the choices that Jacob made rational, logical or honest. I could be wrong, this book being the most experience I have reading about the syndrome. Other than that, I enjoyed the book a lot. I also don't know if I subscribe to the vaccinations/autism theory (that it seems pretty obvious that Picoult buys into - I am not making a sweeping denial, just saying that I haven't really looked at the data), but I love her writing, and I always love her characters, even the flawed ones. I felt very deeply for the mother in this story, like I always do.
