Reading required for my graduate program. In terms of fault diagnosis this book explores the root cause(s) for human error. I do appreciate Sidney Dekker's explanation; often human error is really a systemic error. Various real-life examples spring to mind while listening to the book.

I do question if genuine human error can be fully eliminated? I understand there is fault in the Bad Apple theory although are there some cases human error really is the result of a "Bad Apple"? An individual that is insubordinate and has the desire to cause disruption. Mr. Dekker operates form an inherently good perspective regarding people. I think like, other things, inherently good people might be on a bell curve. Most people are inherently good although occasionally, there truly are some "bad apples". Overall I agree with Mr. Dekker. This is one of those psychology books providing proof to a theory that seems obvious but requires evidence for people to embrace the reality and not follow false science.
