Use of the R slur and consistent picking on a character for being fat, which is meant to be justified by him being a bad person. Of course, this also means she is demonizing a fat man. Joshua comes from a family of doctors and studied to be one himself before choosing not to, they go into some unpleasant medical details about the human body. The protagonist also gets sick and is treated by a doctor in one bit. I also feel it's good to bring up that Joshua gets controlling and takes her phone away in one scene.
