At the heart of the book is the philosophical question - if you knew you had 24 hours to live, what would you do? And as you can expect, there's many ways to go about it. You could stay inside, or you could go out and live. They Both Die at the End covers both of these extremes coming together in an unlikely way. (It also covers other perspectives in side-stories)
The one thing that has always slightly bugged me about Silvera's books is the way that each of his books I've read end. Essentially one of the main characters makes a realisation about themselves or the situation they're in, and then the book abruptly ends. In They Both Die at the End, it's actually used well. It could have been drawn out, but in this instance it's so powerful.
